Guidelines for Patient Self-Adjustment of Evening Insulin


Begin with a dose of _____ units of NPH insulin administered just before bedtime.

If your pre-breakfast blood glucose level is greater than 140 mg/dl for 3 days in a row, then increase your evening NPH insulin dose by _____.

If your pre-breakfast blood glucose level is less than 100 mg/dl for 2 days in a row, then decrease the evening NPH insulin by _____ units.
